Senior Wellness Outreach Program (SWOP)
 
This newly developed program will build upon and complement PGH’s Senior Support Program to bring additional support services to isolated seniors in the two most vulnerable communities in the agency’s catchment area, Salisbury and Newburyport.
 
The Pettengill House has seen an increase in need for at-risk and vulnerable, low income seniors. The number of senior clients receiving case management has increased 43% from FY 2015 to 2016, and an additional 19% from FY 2016 to FY 2017. SWOP will offer its participants support and intervention through a social work model by providing formalized intake meetings, assessments, individual action plans, comprehensive and intensive case management, collaboration efforts and access to community resources. SWOP will strive to decrease isolation among the population served, and break down barriers for accessing appropriate services.
