Family Stabilization/Basic Needs Assistance Program

In order to quality for this program participant must be a client of The Pettengill House Inc. and have completed intake process in current calendar year 

  • Documentation for Emergency Assistance request(vendor bill, lease, landlord letter, mortgage statement)

     

  • Copy of most recent bank statements (checking, savings, money market)

     

  • Evidence of Residency (current utility bill, lease, mortgage)

     

  • Documented evidence of total household income.  This includes the following:

  • Employment Income (letter from employer on company letterhead or copies of 4 most recent pay stubs).

  • Documentation re: child support and alimony (court order and 4 weeks documented).

  • Unemployment Benefits

  • Social Security Benefits/Social Security Disability Benefits

  • AFDC (Welfare) Income  (cash benefits only)

  • Veterans Benefits

  • Workmen’s Compensation

  • Retirement/Pension

  • Dividends/Interest

  • Net Rental Income

  • Other

  • If you file taxes each household member must also provide latest Federal Tax Return

Please note:  After required paperwork is submitted a 10-14 day process time may be necessary for completion
